How to see a doctor
●Persons eligible for the Shinjuku City health checkup
1 Prepare a health checkup form.
2 Select a medical institution from the list of medical institutions above and directly inquire about health checkups and hepatitis virus screenings.
3 On the day of the checkup, receive the hepatitis virus checkup form at the medical institution and fill in the questionnaire.
4 Have a medical examination at the same time as the health checkup.
●Those who are not eligible for the Shinjuku City health checkup
1 Prepare a hepatitis virus checkup form.
(If you do not have one, we will order one from the ward.)
2 Select a medical institution from the list of medical institutions above and inquire directly about receiving a hepatitis virus screening.
3 Fill in the questionnaire and submit the hepatitis virus checkup form to the medical institution on the day of the checkup.
4 See a doctor (blood sampling)
