Pregnant woman medical examination expense subsidy associated with multiple pregnancies

From October 1, 2021, Shinjuku Ward will additionally subsidize up to 5 prenatal checkups for those with multiple pregnancies.
Due to multiple pregnancies, after April 1, 2021, we will subsidize part of the expenses required for prenatal health checkups at your own expense beyond the 14 prenatal health checkup votes.

Target audience

Those who meet all of the following requirements on the day of the prenatal medical checkup after April 1, 2021
[1] Persons registered as residents of Shinjuku Ward
[2]Those who are pregnant with multiple births

Subsidy limit

Up to 5,070 yen per pregnant woman health checkup
Up to 5 times per pregnant woman

* This applies to the 15th to 19th checkups at your own expense after using all 14 prenatal checkup tickets that have already been issued.

Excluded expenses

・Expenses for the 1st to 14th prenatal checkups
・Expenses for pregnant woman health checkup before issuing maternal and child health handbook
・ Medical treatment covered by medical insurance
・Expenses for ultrasonography and cervical cancer screening not associated with medical examinations
・Expenses that do not correspond to the examination items on the medical examination slip, such as document fees, teaching materials fees, vaccination fees, etc.
・Expenses for pregnant woman health checkups taken outside Japan

Application method

Please submit the following documents by visiting the counter of the Health Promotion Division or by mail.
[1] Pregnant woman health checkup subsidy application for multiple pregnancies
[2] A copy of the front page of the Maternal and Child Health Handbook (for the number of children with multiple births)
[3] A copy of the page showing the progress of the pregnancy in the Maternal and Child Health Handbook (pages 8 and 9; 1 sheet is acceptable)
[4]Original receipt and statement of the relevant prenatal checkups (15th to 19th)

Application period

One year from the date of the last prenatal health checkup or from the date of delivery

(Example) If October 1, 2021 is the date of birth
The application and attached documents must be received by the Health Promotion Division by September 30, 2022 (must arrive).
