What is community-based service?

Community-based services are long-term care insurance services that enable elderly people, including those with dementia, to live with peace of mind in their familiar communities even if they require support or long-term care.
Currently, there are all seven types of community-based services in the city. The types and contents of services are shown in the table below.
We issue leaflet "what is community-based service?" which explained community-based service clearly in city. This time, in order to further promote the use of services, we have created a revised version that includes examples of use received from each service provider. In addition, we have compiled examples of use that could not be included in the leaflet, so please take a look at them as well.

Regular patrols and occasional home-visit nursing care
You can receive regular visits through close cooperation between nursing staff and nurses. In addition, you can receive support at any time by reporting or calling.

Night-time home-visit nursing care
There are home-visit nursing care, which provides nursing care through regular visits at night, and home-visit nursing care, which provides nursing care at the request of the user, such as in an emergency.

<For those who need services that commute during the day>
Community-based outpatient nursing care
A small day service center with a capacity of less than 19 people, where you can receive nursing care such as eating and bathing and functional training on a day trip.

<For those who need daytime dementia care>
Day care for dementia (day care for preventive dementia)
People diagnosed with dementia can receive care and support such as eating and bathing, and functional training on a day trip.
<For those who need a combination of commuting/visiting/overnight services>
Small-scale multi-functional in-home care (preventive long-term care small-scale multi-functional in-home care)
You can receive services such as “commuting” to a small residential facility, “visiting” to have someone come to your home, and “staying” at a facility.

Nursing small-scale multifunctional home care
In addition to commuting to small residential facilities, visiting homes, and staying at facilities, home-visit nursing services are available, depending on the situation of the user. You can receive a service that combines

Cohabitation care for dementia (cohabitation care for preventive dementia)
[Group home for the elderly with dementia]
People diagnosed with dementia live together in a small group while receiving nursing care and support such as eating and bathing, and functional training. The user aims to continue the life they have lived so far as much as possible in the communal life.
