After the examination, a user registration decision notice will be mailed to your home.
* If you have lost your user registration decision notification and wish to have it reissued, please submit the following application form by mail or at the counter.
3. Make a reservation at the facility
Please check the terms and conditions of each facility and make a reservation directly with the facility.
At the time of reservation, please be sure to tell us the registration number described in the usage registration decision notification.

Cancellation
- For cancellations, please contact Minato Public Health Center Health Promotion Division Regional Health Section and the implementing facility at least 3 days before the start date of use (if it falls on a closed day, by the open agency day immediately before the closed day).
- Cancellation methods vary depending on the facility. Be sure to confirm directly at the time of booking.
- If you do not contact the facility you reserved by the deadline and do not use it, you may be required to pay a user fee. In addition, you cannot apply or use it again.
4. Usage decision
Please contact the Minato Public Health Center after confirming your reservation for using the facility. After that, a business use confirmation will be issued.
If you have lost your business use confirmation and wish to have it reissued, please submit the following application form by mail or at the counter.
