Kita Ward issues the following documents to those who have been diagnosed as positive for the new coronavirus infection.

Please note that even if you are a resident of Kita Ward, the Kita Ward Public Health Center will not be able to issue a certificate if the public health center in charge is different.

Hospitalization recommendation letter, public expense decision notice

We are only mailing to those who have been hospitalized with a new corona virus infection.

If the period of hospitalization is extended, we will also send you a "Recommendation for Extension of Hospitalization" in addition to the above.
