Since 2008, Chiyoda Ward has created the "Chiyoda Ward Safe Living Monitoring Ledger" and is working to ensure that the elderly can live with peace of mind. This time, as a new service, we will distribute "watching stickers" and "medicine notebook covers" that can be attached to watching stickers.
This initiative was made possible through an exchange of opinions with pharmacists who work at pharmacies that are usually visited by the elderly. Even if a pharmacist noticed an abnormality in an elderly person who visited them when delivering medicine, they were unable to provide information to related organizations such as the ward office due to personal information concerns. Therefore, by attaching a "watching sticker" with a number assigned to each individual to an item that is used on a daily basis, such as a medicine notebook, you can quickly and appropriately respond to emergency situations by contacting the number. It can be expected that this will lead to a better response.
This is the first initiative in the 23 wards to distribute a "medicine notebook cover" along with a "watching sticker". Thank you very much for your introduction.

If support starts from looking for a person who is in need of assistance, time may be lost and prompt assistance may not be provided.Therefore, keep the patient registration card and the business card of the care manager of the long-term care insurance system in the "medicine notebook". I thought it would be great if there were cases where this could be done, and I could put a watch sticker on it so that I could gently watch over the elderly on a daily basis. We hope to contribute to the development of communities where the elderly can live with peace of mind. ”
