This is a system that subsidizes medical expenses for people aged 18 and over with physical disabilities to reduce or improve their physical impairments so that they can better adapt to their daily lives and work. You have you make designated statement of opinion in medical institution receiving designation as independence support medical institution beforehand, and please apply.

2 What you need for the application procedure

Copy of health insurance card (for all family members who have the same insurance as the target patient)
Municipal tax taxation certificate or ex-officio investigation consent form for municipal and prefectural tax (If you do not have an address in Shizuoka City as of January 1, please request a municipal tax taxation certificate from the municipality where you used to live.)
Documents that show public pensions, pensions, and various allowances (certificates, transfer notices, or transfer passbooks for pensions, etc.) *

*For 6 and 7 income, please submit the income from the year before the previous year if you apply from January to June, and the previous year's income if you apply from July to December.
3 Notes
It takes about one month from application to payment. Medical expenses that have not yet been determined to be paid are not eligible, so please apply at least 1-2 months before the start date of use. (If you cannot apply before the start date of medical care due to emergency surgery, etc., please contact us.)
Please note that if you are receiving a subsidy for medical expenses for severely mentally and physically disabled people, the burden including the refund will be the same even if you use this system.
Please note that depending on the tax situation of the household, the same amount may be determined as the high-cost medical care benefit, receipt for specified disease treatment, or certificate of application of maximum amount.
4 Self-pay
The "household" used to determine your income category is based on your medical insurance. Family members with different medical insurance policies are considered separate households.
