1 Overview of the system
It is a system that subsidizes medical expenses of medical insurance and the cost of medicines, etc. for those who are eligible.
At hospitals and pharmacies, you will be asked to pay the full amount of medical expenses and other health insurance medical expenses that you have to pay yourself, and we will provide assistance at a later date.
* In general, the transfer will be made three months after the treatment month, but if payment is delayed or you fall under high-cost medical expenses,
Or, due to the circumstances of the medical institution, etc., it may not be paid after 3 months.
The subsidy is calculated by deducting the copayment amount of 500 yen per medical institution per month from the copayment amount.
・At the same general hospital, you will be charged 500 yen per month even if you visit multiple departments, but you will have to pay another 500 yen per month for dentistry.
・ If your copayment is less than 500 yen, you will not be subsidized.
・ There is no copayment for medicines. (Please pay the full amount of your co-payment at the window, and we will subsidize it at a later date.)
(Note) If you are eligible for medical expense subsidies with a specific disease medical care beneficiary certificate, self-reliance support medical care, etc., or medical expense subsidy with a specific disease medical care beneficiary certificate (dialysis, etc.), the maximum amount for each is covered. 3 About eligible medical expenses

5 About the method of subsidy
Below are ways to help.
1. Automatic reimbursement method When visiting a medical institution in the prefecture, present your recipient's certificate (excluding massage, acupuncture, and moxibustion) and pay the co-payment for medical treatment covered by insurance. We will automatically deposit the money into your registered account.
2 Reimbursement application method (Medical expense subsidy payment application)
In the following cases, please apply at each Ward Welfare Office Disabled Persons Support Section or each Health and Welfare Center (excluding Kanbara).

6 Transfer of subsidies

8 High-cost medical expenses, additional benefits, etc.
If you receive high-cost medical expenses, additional benefits, etc. from your health insurance, we will deduct the relevant amount from the subsidy and pay the subsidy.
Please confirm your income category and co-payment limit with your health insurance, and if you are eligible for high-cost medical care, be sure to apply for payment and submit a notification of decision.
*If you are enrolled in Shizuoka City National Health Insurance or the medical system for the elderly aged 75 or over, you do not need to submit this form.
(Note) Please note that the payment of the subsidy may be temporarily suspended if there is no submission .

By presenting the “Specified Disease Medical Care Recipient Certificate” at the counter, the maximum amount of self-payment for the disease will be 10,000 yen or 20,000 yen per month for each medical institution.
If the sum of the amount paid to the medical institution and the amount paid at the pharmacy for out-of-hospital prescriptions exceeds the maximum copayment amount, the excess amount will be paid as high-cost medical expenses from the enrolled health insurance. Don't forget to apply for health insurance.

10 Certificate of Recipient of Medical Expenses for Persons with Severely Disabled Persons
Toward the target, we issue "certificate of severely mentally and physically disabled medical expense subsidy recipient" by application.
As a general rule, the expiration date of the recipient certificate is from October 1 of the year to the end of September of the following year. (There are some exceptions.)
If you continue to be eligible, we will automatically send you a new recipient certificate. (No procedures are required.)
Please note that if you are 65 years of age or older, your eligibility for the hospitalization expenses subsidy may change at the time of renewal depending on the taxation status of your household.
If you do not receive a new recipient identification card by the end of September, please contact the Disabled Persons Support Promotion Division (054-221-1587).
