Physically and mentally disabled dependent mutual aid system

A guardian who supports a person with a disability pays a fixed monthly premium during his or her lifetime, so that if the guardian dies or is severely disabled, the person with a disability is covered. This is a voluntary enrollment system that provides a fixed amount of pension for life.

Enrollment requirements

1. Parental requirements

Parents (parents, spouses, brothers and sisters, grandparents, other relatives, etc.) currently supporting a person with disabilities who meet all of the following requirements.

Must have an address in Shizuoka City.
You must be under 65 years of age as of April 1st of the fiscal year (April 1st to March 31st of the following year) at the time of enrollment (at the time of addition of units in case of additional units).
For example, if you turn 65 on April 5th, you will be 64 years old as of April 1st, so you can enroll until March of the following year.
You must be in good health to be eligible for a life insurance policy without any specific illness or disability.
Depending on your health condition, you may not be able to participate in this system.
Only one guardian can be enrolled for each person with a disability.

2. range of people with disabilities

Persons with disabilities who fall under any of the following categories and are recognized as having difficulty in supporting themselves independently in the future. (Age does not matter.)

Intellectual disability
Possessing a certificate of the physically disabled and whose disability falls under grades 1 to 3
Persons with permanent mental or physical disabilities (psychosis, cerebral palsy, progressive muscular atrophy, autism, hemophilia, etc.) with the same degree of disability as (1) or (2) Those who are recognized as degree

1. amount of premium

The amount of the premium is determined according to the subscriber's age as of April 1 of the fiscal year (April 1 to March 31 of the following year) at the time of enrollment (when the number of units is added in the case of additional units).

2. Contribution exemption

Please make payments until both of the following "Requirement 1" and "Requirement 2" are met.
If both requirements are met, no further payments are required.

Requirement 1	20 years from the date of subscription (date of additional units for additional units)
Requirement 2	The period from the date of enrollment (the date of additional enrollment in the case of additional enrollment) to the day before the anniversary date of enrollment in the fiscal year in which the participant is 65 years old as of April 1 (from April 1 to March 31 of the following year).

For enrollees, pension administrators and their families ~Regarding procedures after enrolling~

If any of the following situations occur after enrolling, please immediately contact the Welfare Office for Persons with Disabilities in the ward where you live.

When the subscriber dies or becomes severely disabled.
When the disabled person dies before the subscriber.
When a participant withdraws from this system.
When a member moves to another city/prefecture and withdraws from the system of Shizuoka City.
When the address or name of a subscriber, person with a disability, or pension administrator changes.
When the pension administrator dies, or when you appoint or change the pension administrator.
When there are unclear points due to other changes, etc.

For those who are receiving a pension - Procedures while receiving a pension -

If you have started receiving pension benefits due to the death or severe disability of a participant, and if any of the following events occur, please contact the Disabled Persons Support Section of the welfare office of the ward where you live.

if the pensioner dies;
When a pensioner's address or name changes.
When the pension administrator dies, or when you appoint or change the pension administrator.
