If you are visiting a hospital in the city, please bring a medical certificate from a doctor designated by the mayor of Shizuoka City.
If you do not know who the designated doctor is, please contact the Shizuoka City Regional Rehabilitation Promotion Center or the Life Support Section of the welfare office of each ward.

As long as the face is clearly recognizable, the photo does not have to be a photo for identification purposes.
(However, please remove hats and sunglasses.)
The photo paper is limited to thick paper such as photographic paper.
3 From application to delivery
(1) Please prepare the necessary documents and submit them to the Welfare Office for Persons with Disabilities in your ward.
What to bring (1) Application form (2) Doctor's certificate (Must be written by a designated doctor.)
(3) 1 photo (length 4cm x width 3cm)
(2) After 1-2 months, we will notify you by post to the location of your choice.
(3) Gather the necessary items and visit the welfare office of the ward where you live.
We will hold a briefing session.
