When a person with intellectual development delay wants to receive various services, it is necessary to receive a "rehabilitation notebook" issued by the mayor of Shizuoka City based on the application of the person. ◆ Mental Disability Certificates are issued to those with mental disabilities. Rehabilitation notebooks are judged at the Shizuoka City Child Guidance Center or the Shizuoka City Regional Rehabilitation Promotion Center.

The photo paper is limited to thick paper such as photographic paper.
2 From application to delivery
(1) Please prepare the necessary documents and submit them to the Welfare Office for Persons with Disabilities in your ward.
At that time, an interview date will be determined.
What to bring: (1) Application form (2) 1 photo (length 4cm x width 3cm)
(2) Interview and decision will be made at the Child Guidance Center or Regional Rehabilitation Promotion Center.
(3) After 1-2 months, we will notify you by post to the location of your choice.
(4) Gather the necessary items and visit the welfare office for the disabled in your ward.
I will give you the notebook.
