Overview of planning consultation support and consultation support for children with disabilities

Due to the partial revision of the Act on Support and Support for Persons with Disabilities and the Child Welfare Act in April 2012, all users who use welfare services for persons with disabilities and outpatient support for children with disabilities are requested to submit a proposal for the use of services (or support for children with disabilities). I decided to create a draft plan.
Submission was not mandatory until March 2015 due to transitional measures, but due to the end of transitional measures, submission of service usage plan (or handicapped child support usage plan) is required from April 2015. have become.
When creating a plan for the use of services, etc., a designated specific consultation support office (or a designated consultation support office for children with disabilities) will assist in the preparation, or a so-called "self-plan" will be prepared.
