Local life support project (mobility support, daytime temporary support)

Mobility support
For people with disabilities who have difficulty moving outdoors, guide helpers who are qualified as mobility support workers provide support for going out based on a plan that suits the individual user. In order to enrich the user's life in the community, the purpose is to support going out that is essential for social life, leisure activities, and going out related to social participation.
<Target people>
(1) Persons with physical disability certificate 1st grade (both upper and lower limbs are 1st grade)
(2) Those who have a rehabilitation notebook or those who are recognized by the city as equivalent to those who have a rehabilitation notebook
(3) Those who have a mental disability health and welfare notebook
This is a project that provides temporary monitoring and other support during the day. The aim is to secure a place for activities during the daytime for persons with disabilities, provide opportunities for creative activities, etc., and support temporary rest for family members who provide daily nursing care.
<Target people>
(1) Those who have an address in the city and have a physical disability certificate
(2) Those who have an address in the city and have a rehabilitation notebook, or those who are recognized by the city as equivalent to those who have a rehabilitation notebook
