We would like to inform you that the Ministry of Health, Labor and Welfare has requested that medical certificates ("Clinical Survey Individual Form" for designated intractable diseases and "Medical Opinion Form" for specified chronic childhood diseases) be made available online.

About information on designated intractable diseases and childhood specific chronic disease database update

Currently, the Ministry of Health, Labor and Welfare is renewing the system of the database for intractable diseases and ignorance, which is scheduled to be released after 2022, in order to make medical certificates online. Please read through it as it includes the overall picture of the new system, the database construction schedule, and matters to be handled by medical institutions.

*Medical institutions that use their own medical systems are expected to require system upgrades.
