This report will be an important basic document for future health, labor and welfare administration. In addition, if you do not submit a notification, your name will not be posted on the "Search System for Confirmation of Qualifications for Physicians" and "Search System for Confirmation of Pharmacist Qualifications". Even if you are not working as of December 31, 2022, please do not forget to submit the notification form.

Fill in the necessary information on the prescribed notification form and, in principle, submit it to the Ward Health and Welfare Center in your place of residence or place of employment .

If you have more than one place of employment, please fill in the main place of employment and submit it. In addition, please make sure that there are no omissions in the items on the notification form (especially address, name, phone number).

Notification forms will be distributed from mid-December at the Health and Medical Affairs Division of the Osaka City Public Health Center and the Health and Welfare Centers in each ward of the city .
